  1. Background Information: What are your experiences? Where are you located? What is your training background? Have you given a presentation like this before? Etc.
  2. Workshop Title(s) and Description(s): List the title(s) of workshops you can offer, as well as brief descriptions of the workshops.
  3. Logistics: Descriptions of the length generally allocated to specific workshops, or if it is flexible mention that.
  4. Audience: Please describe the audience that your workshop is geared towards. For example, "How to talk about the New Church" workshop would be geared towards people currently involved in New Church congregations seeking tools for sharing their faith.
  5. Workshop Type: Is it interactive? PowerPoint? Slides? Lecture style? Hands-on? Etc.
  6. Please include your contact information.
  7. Please send a digital photo of yourself, or a printed photo that will be scanned and returned to you promptly.
